2024 Consent for RAD Exams *

I have read the 2024 Exam Information Sheet (emailed and linked below) and I would like to enter my child in an RAD exam/award in 2024. I understand that on receipt of this form, Barbara Todd Ballet will email me an invoice for the RAD exam/award fee PLUS the RAD exam programme fee as detailed on the information form. If the RAD entry fee and programme fees are not paid, I understand that my child will be unable to sit their exam. I confirm that if my child leaves Barbara Todd Ballet before the exam, their entry for this RAD exam session will be forfeited. I understand that it is important that my child aims to attend all classes (regular classes plus Saturday exam classes as per the schedule).
